Two weeks after the passing of legendary Black Sabbath frontman Ozzy Osbourne, the official cause of death has been confirmed. According to a death certificate obtained by The New York Times, Osbourne died as a result of cardiac arrest outside of a hospital setting, with underlying medical conditions including coronary artery disease and Parkinson’s disease contributing to his death.

The certificate, filed in London by Osbourne’s daughter, Aimee Osbourne, listed the 75-year-old’s primary occupation as “Songwriter, Performer and Rock Legend.” The official cause of death is detailed on the certificate as: “(a) Out of hospital cardiac arrest; (b) Acute myocardial infarction; (c) Coronary artery disease and Parkinson’s disease with autonomic dysfunction (Joint Causes).”

Osbourne died on the morning of July 22 at his family’s home in the village of Chalfont St. Giles in Buckinghamshire, England. Emergency medical services, including an air ambulance, were dispatched to the residence, but he was pronounced dead shortly after their arrival. His family publicly announced his death later that day, though no cause was initially given.

“It is with more sadness than mere words can convey that we have to report that our beloved Ozzy Osbourne has passed away this morning,” the Osbourne family said in a statement released at the time. “He was with his family and surrounded by love. We ask everyone to respect our family’s privacy at this time.”

Osbourne had long been open about his declining health. In 2020, he disclosed that he had been diagnosed with a form of Parkinson’s disease known as PRKN 2, which can affect motor function and lead to a variety of symptoms, including muscle stiffness and tremors. In the years that followed, he underwent several surgeries related to spinal and nerve issues and had been seen using mobility aids. In February 2025, Osbourne confirmed in an interview that he had lost the ability to walk independently.

Despite his health challenges, Osbourne remained musically active until shortly before his death. Earlier in July, he made a surprise appearance at a Black Sabbath reunion concert, joining the band’s original lineup for what became a final farewell performance. Performing while seated on a throne, he delivered renditions of classic tracks such as “Iron Man” and “War Pigs,” marking an emotional moment for fans and bandmates alike.

The performance was widely seen as a poignant return for the singer, whose influence on heavy metal and rock music is considered foundational. Osbourne first rose to prominence in the early 1970s as the frontman of Black Sabbath, a band that would go on to define the sound and aesthetics of heavy metal for decades. After being dismissed from the group in 1979, he launched a successful solo career that yielded hits such as “Crazy Train,” “Mr. Crowley,” and “No More Tears.”

Known as much for his theatrical persona as for his music, Osbourne became a pop culture icon through his public antics and his reality television series The Osbournes, which aired in the early 2000s. He was inducted into the Rock and Roll Hall of Fame with Black Sabbath in 2006 and has remained a beloved figure in the global music community.

While Osbourne had stepped back from touring in recent years due to his health, he continued to record and release music, including the critically acclaimed 2022 album Patient Number 9. That record, which featured collaborations with Jeff Beck, Tony Iommi, and Eric Clapton, marked a creative resurgence late in his career.

Ozzy Osbourne is survived by his wife, Sharon Osbourne, and his children Aimee, Kelly, and Jack. Plans for a public memorial or tribute concert have not yet been announced.
